How can we become part your medical healthcare team?
STEP 1: Call our office at 415-665-4953 to speak with Kevin McCarthy, Office Manager. Alternatively, you may streamline the enrollment process by faxing to us at 415-665-4953 the completed form listed below. The health information you provide will help us better understand how we may help you. The financial information you provide will help us help you better understand your Medicare Part B coverage or private pay obligation for physical therapy services for this year so there are no financial surprises.
STEP 2: Once we all understand your healthcare needs and financial healthcare coverage (i.e. Medicare Part B, secondary insurance, private pay), we will send you our practice enrollment forms which gives Dr. McCarthy permission for the examination and treatment, and for sharing protected information with others you deem are involved in your care.
STEP 3: When all enrollment paperwork has been completed, then Dr. McCarthy will call you to schedule the first appointment in your home.
